NFL Week 4 brings several significant injury questions for fantasy football managers, with Justin Jefferson, Puka Nacua, Jayden Daniels, Nico Collins and Mike Evans among the key players to monitor heading into this week's games.

Here are the latest injury updates, practice statuses and Week 4 outlooks for the top fantasy football players dealing with injuries.

Justin Jefferson, WR Vikings - Ankle

Jefferson suffered an ankle sprain during the Vikings' Week 3 win over the Buccaneers. He left the game and did not return, although head coach Kevin O'Connell told reporters he was close to returning. In Jefferson's absence, Jordan Addison stepped up, finishing with five receptions for 90 yards and a touchdown. On Wednesday, Jefferson did not participate in practice. The team considers him day-to-day.

The Vikings take on the 0-3 Miami Dolphins, who are allowing 231 passing yards per game and have surrendered seven passing touchdowns.

Minnesota's offense has struggled this season with Kyler Murray and Carson Wentz under center, ranking last in the NFL in both total offense and passing offense. Jefferson and Murray have played only 17 snaps together this season, adding another layer of uncertainty to Jefferson's Week 4 fantasy outlook.

Puka Nacua, WR Rams - Groin

Nacua has missed the previous two games due to hip and groin injuries related to the psoas issue he has dealt with since training camp. He was able to suit up in Week 1 against the 49ers, finishing with five receptions for 74 yards. Head coach Sean McVay said he expects Nacua to play in Week 4 against the Eagles. On Wednesday, Nacua returned to practice in a limited capacity.

In his absence, Davante Adams has been the biggest fantasy beneficiary. In the two games without Nacua, Adams totaled 15 receptions for 332 yards and two touchdowns. In Week 1 with Nacua, Adams recorded just three catches for 26 yards.

Last season, Nacua was Matthew Stafford's primary target when he and Adams were on the field together, while Adams was Stafford's primary red-zone target, finishing with a league-leading 14 touchdowns. When healthy and on the field, Nacua has been a top-tier fantasy option, while Adams' fantasy value alongside Nacua has been more touchdown-dependent.

Jayden Daniels, QB Commanders - Elbow

Daniels missed Week 3 after dislocating his left elbow again in Week 2 against the Cowboys. He elected not to undergo surgery and will wear a brace upon his return. Daniels returned to practice Wednesday and traveled with the team to London ahead of the Week 4 game.

In his absence, Marcus Mariota has filled in. Mariota made eight starts for Washington during the 2025 regular season, posting a 91.3 passer rating with 1,602 passing yards, 10 touchdowns and five interceptions. Last week against Seattle, one of the NFL's top defenses, Mariota threw for 183 yards and three touchdowns.

Mariota has also shown a solid connection with Terry McLaurin. Across 7 total career games where McLaurin played and Mariota was the starting quarterback, McLaurin has caught 33 receptions on 51 targets for 485 yards and 5 touchdowns. Last week, McLaurin finished with six receptions for 77 yards and a touchdown. If Daniels remains out in Week 4, Mariota's track record with McLaurin provides some encouraging fantasy context for Washington's top receiver.

Nico Collins, WR Texans - Hamstring

Collins returned to practice Wednesday as a limited participant for the first time since sustaining a Grade 1 hamstring strain in Week 1. Collins has missed the Texans' past two games. Head coach DeMeco Ryans said the team is "hopeful" he will play Sunday.

When healthy, Collins is one of the league's most productive wide receivers. In 2025, Collins played 15 games, finishing with 71 receptions for 1,117 yards and six touchdowns. Stroud has also performed significantly better with Collins on the field, posting a 95.1 passer rating with 53 touchdowns and 20 interceptions, more than 10 points higher than his rating without Collins.

Collins' potential return would therefore provide a significant boost not only to his own Week 4 fantasy outlook, but also to Stroud and the Texans' passing offense.

Mike Evans, WR 49ers - Rib

Evans suffered a hard hit to his ribs and oblique during the second quarter of the Week 3 win against the Arizona Cardinals and was carted to the locker room. He did not practice Wednesday as he continues to recover from the rib injury. Evans is considered day-to-day, while the hip injury he dealt with last week is no longer listed on the injury report.

Evans has played in all three of the 49ers' wins this season, totaling 12 receptions for 137 yards and two touchdowns. He ranks as the WR21 in PPR leagues, while his 11.4 yards per catch is a career low.

Evans has dealt with several injuries since training camp, including quad and groin issues before suffering an inguinal hip injury in Week 2. He played through the hip issue in Week 3 before suffering the rib injury against Arizona. Teammate George Kittle said the injury feels like "fractured ribs," making Evans' practice participation and availability important to monitor ahead of Week 4.
