Week 4 delivered a brutal slate of injuries across the league, with several playoff-contending teams facing major setbacks. Giants rookie WR Malik Nabers suffered a season-ending ACL tear, Ravens QB Lamar Jackson’s hamstring scare forced a cautious exit, and the Chargers’ offensive line depth is in crisis after Joe Alt went down. Meanwhile, 49ers QB Brock Purdy continues to battle accuracy issues tied to turf toe, and the Eagles’ offense stalled when RT Lane Johnson left with a stinger/shoulder injury.

From video analysis by Pro Football Doc, here’s what we know so far about these key injuries, and what they could mean moving forward.

Malik Nabers - Giants / ACL Tear

Giants WR Malik Nabers appeared to suffer a right ACL tear on the step before he went down. He was carted off, and later reports confirmed Pro Football Doc's initial diagnosis, “It does appear to be an injury to his right knee consistent with an ACL tear… significant fear that a season is done,” said Pro Football Doc. A devastating blow for the Giants, especially with Jackson Dart starting for the first time in his career.

Lamar Jackson - Ravens / Hamstring

Ravens QB Lamar Jackson left with a hamstring injury, but video review suggests it isn’t severe. The decision to sit him while trailing by three scores was precautionary. “Do you want to turn a mild hamstring into a 3-4 week injury?… Thankfully, hopefully, fingers crossed, don’t believe it’s severe. And there remains open the chance to play this next week,” said Pro Football Doc. No expectation of IR at this point.

Joe Alt - Chargers / Ankle

The Chargers are dealing with cluster injuries along the offensive line. LT Rashawn Slater and RG Mekhi Becton were already out, forcing rookie Joe Alt to flip to left tackle. But after suffering a high ankle and possible MCL injury, Alt is now sidelined too — leaving the Chargers down to LT3, RG2, and RT2. “That’s a cluster injury… Herbert didn’t have the protection he needed, and that’s why we said the Giants have a chance in this game,” said Pro Football Doc.

Brock Purdy - 49ers / Turf Toe

49ers QB Brock Purdy is clearly not at full strength. By video, his right turf toe — on his back plant foot — is affecting accuracy and consistency. Normally a 70%+ passer, he’s been missing throws and failing to drive off his back foot. “From what I’ve seen of Brock Purdy, he is nowhere near 100%… It is definitely, according to our video study, Brock Purdy’s back plant foot turf toe, and it is affecting his accuracy to a degree,” said Pro Football Doc.

Lane Johnson - Eagles / Shoulder

Eagles RT Lane Johnson left with what was reported as a shoulder injury, though it may have been another stinger. His absence immediately stalled the Philly offense against Tampa. “There’s a suspicion there that stinger again… They didn’t move the ball much after Lane Johnson left the game. He’s been a key for them,” said Pro Football Doc. This will be one to watch closely heading into Week 5.
