Is Dry Needling Capable of Causing a Collapsed Lung?
Dr. Chao made it clear he remains skeptical that dry needling caused Watt’s lung injury.
Dry needling is commonly used to relax muscle knots and tightness. While the technique involves inserting thin needles into muscle tissue, Dr. Chao explained that the depth required to puncture a lung -especially in an athlete like T.J. Watt - is extreme.
“You’d have to bury that needle incredibly deep,” Chao said, noting Watt’s significant muscle mass in his chest, thorax, and trapezius areas. Even then, the diameter of dry-needling needles is extremely small, comparable to acupuncture needles and much thinner than needles used for NFL rib blocks.
During his career, Dr. Chao personally performed dozens of rib block injections using larger 25-gauge needles and never caused a collapsed lung.
For dry needling to be the cause, Chao explained, the needle would have to be inserted very deeply, repeatedly, and aggressively, which he finds unlikely.

Dr. Chao questioned whether the procedure involved a chest tube - commonly used to re-expand a lung - or an actual surgical repair of lung tissue.
“If you sew or resect lung tissue,” Chao explained, “you’re creating a larger defect than what a dry-needling needle would cause.”
This makes a direct cause-and-effect link between dry needling and the lung collapse medically questionable.
The More Likely Cause: A Ruptured Lung Bleb
Dr. Chao reiterated that the most plausible explanation remains a ruptured lung bleb.
A bleb - a small air blister or cyst in the lung - can rupture spontaneously, particularly in young, highly active athletes under intense physical stress. Watt had reportedly been weightlifting and exerting himself, which increases internal chest pressure.
While rare, spontaneous bleb ruptures do occur, even in otherwise healthy athletes.
In this scenario, dry needling may be coincidental or extraneous, rather than causal.
